RESUMO

Proximal femur geometry is an important risk factor for diagnosing and predicting hip and femur injuries. Hence, the development of an automated approach for measuring these parameters could help physicians with the early identification of hip and femur ailments. This paper presents a technique that combines the active shape model (ASM) and deep learning methodologies. First, the femur boundary is extracted by a deep learning neural network. Then, the femur's anatomical landmarks are fitted to the extracted border using the ASM method. Finally, the geometric parameters of the proximal femur, including femur neck axis length (FNAL), femur head diameter (FHD), femur neck width (FNW), shaft width (SW), neck shaft angle (NSA), and alpha angle (AA), are calculated by measuring the distances and angles between the landmarks. The dataset of hip radiographic images consisted of 428 images, with 208 men and 220 women. These images were split into training and testing sets for analysis. The deep learning network and ASM were subsequently trained on the training dataset. In the testing dataset, the automatic measurement of FNAL, FHD, FNW, SW, NSA, and AA parameters resulted in mean errors of 1.19%, 1.46%, 2.28%, 2.43%, 1.95%, and 4.53%, respectively.

OBJECTIVE: Diagnosis of small airway disease on computed tomography (CT) scans is challenging in patients with a history of chemical warfare exposure. We developed a software package based on different methodologies to identify and quantify small airway disease in CT images. The primary aim was to identify the best automatic methodology for detecting small airway disease in CT scans of Iran-Iraq War victims of chemical warfare. METHODS: This retrospective case-control study enrolled 46 patients with a history of chemical warfare exposure and 27 controls with inspiratory/expiratory (I/E) CT scans and spirometry tests. Image data were automatically segmented, and inspiratory images were registered into the expiratory images' frame using the locally developed software. Parametric response mapping (PRM) and air trapping index (ATI) mapping were performed on the CT images. Conventional QCT methods, including expiratory/inspiratory mean lung attenuation (E/I MLA) ratio, normal density E/I (ND E/I) MLA ratio, attenuation volume Index (AVI), %low attenuation areas (LAA) < -856 in exhale scans, and %LAA < -950 in inhale scans were also computed. QCT measurements were correlated with spirometry results and compared across the two study groups. RESULTS: The correlation analysis showed a significant negative relationship between three air trapping (AT) measurements (PRM, ATI, and %LAAExp < -856) and spirometry parameters (Fev1, Fvc, Fev1/Fvc, and MMEF). Moreover, %LAAExp < -856 had the highest significant negative correlation with Fev1/Fvc (r = -0.643, P-value < 0.001). Three AT measurements demonstrated a significant difference between the study groups. The E/I ratio was also significantly different between the two groups (P-value < 0.001). Binary logistic regression models showed PRMFsad, %LAAExp < -856, and ATI as significant and strong predictors of the study outcome. Optimal cut-points for PRMFsad = 19%, %LAAExp < -856 = 23%, and ATI = 27% were identified to classify the participants into two groups with high accuracy. CONCLUSION: QCT methods, including PRM, ATI, and %LAAExp < -856 can greatly advance the identification and quantification of SAD in chemical warfare victims. The results should be verified in well-designed prospective studies involving a large population.

Computed tomography (CT) imaging is being increasingly used in clinical practice for detailed characterization of lung diseases. Respiratory diseases involve various components of the lung, including the small airways. Evaluation of small airway disease on CT images is challenging as the airways cannot be visualized directly by a CT scanner. Small airway disease can manifest as pulmonary air trapping (AT). Although AT may be sometimes seen as mosaic attenuation on expiratory CT images, it is difficult to identify diffuse AT visually. Computer technology advances over the past decades have provided methods for objective quantification of small airway disease on CT images. Quantitative CT (QCT) methods are being rapidly developed to quantify underlying lung diseases with greater precision than subjective visual assessment of CT images. A growing body of evidence suggests that QCT methods can be practical tools in the clinical setting to identify and quantify abnormal regions of the lung accurately and reproducibly. This review aimed to describe the available methods for the identification and quantification of small airway disease on CT images and to discuss the challenges of implementing QCT metrics in clinical care for patients with small airway disease.

Air pollution particulate matter (PM) is a world risk factor that the effects of long-term exposure to these factors in terms of damage to cardiovascular and pulmonary function are well known, but little is known comparatively about the effects of PM on emotional and cognitive processes. Exposure to PM can adversely affect the central nervous system (CNS) by inflammatory pathways and activation of reactive oxygen species (ROS) associated with urban air pollution PM. Therefore, we investigated whether prolonged exposure to diesel exhaust particles (DEPs) affects hippocampal inflammatory cytokines and emotional and cognition responses. Male mice were exposed to DEPs for 6 and 12 weeks. DEP-exposed mice indicated more disorders in depressive-like responses and spatial memory and learning than in control groups. Pro-inflammatory cytokine expression in tge hippocampus was increased among mice exposed to DEPs. The number of activated microglia increased in the dentate gyrus (DG) and CA1 regions of the hippocampus in DEP-exposed mice. These results show that chronic exposure to DEPs can alter neurobehavioral and impair cognition. Generally, these findings reaffirm the importance of protecting from exposure to ambient PM2.5 and also advance our understanding of the toxic actions of air pollution nanoparticles.

Air pollution is associated with many neurodevelopmental and neurological disorders in human populations. Rodent models show similar neurotoxic effects of urban air pollution ultrafine particulate matter (UFPs < 100 nm (PM0.1)), collected by different methods or from various sources. Exposure to ultrafine particulate matter (UFPs < 100 nm (PM0.1)) can be adversely impacting the central nervous system (CNS) by the activation of proinflammatory pathways and reactive oxygen species associated with air pollution particulate matter. We investigated hippocampal inflammatory cytokines, neurobehavioral alteration, and neuronal morphology following exposure to diesel exhaust particles (DEPs) in mice. Male mice were DEPs exposed for 14 weeks. Mice exposed to DEPs showed more disorders in spatial memory and learning and depressive-like responses than control mice. Expression of hippocampal pro-inflammatory cytokine was increased among DEPs exposure mice. The density of neurons in hippocampus CA1, CA3, and dentate gyrus (DG) regions decreased in DEPs mice. Overall, these findings show that prolonged exposure to DEPs in the world's major cities can alter neurobehavioral and impair cognition.

BACKGROUND: Structural properties of the arterial wall are important diagnostic parameters. The current study aimed at investigating the hemodynamic properties and intima-media thickness changes of the common carotid artery in human subjects with atherosclerosis in order to determine the relationships between these indices. METHODS: This study presented methods to detect instantaneous changes in the lumen diameter, intima media thickness, longitudinal movement and acceleration, and velocity of the left side of common carotid artery. These parameters were measured in 155 male patients, categorized into control (n = 42), mild (n = 39), moderate (n = 37), and severe (n = 37) carotid stenosis groups by B-mode and Doppler ultrasonography. Extracted parameters were used to estimate the biomechanical properties of arteries, including radial strain, arterial stiffness index, Young's elastic modulus, circumferential stress, shear stress, axial stress, critical bent buckling pressure, and critical buckling torque. FINDINGS: All biomechanical parameters of common carotid artery were significantly different in patients with mild, moderate, and severe stenosis, compared to the control group (P < 0.05). Moreover, the current results showed a significant correlation between intima media thickness and non-intima media thickness-based biomechanical indices including circumferential strain, stiffness index, and shear stress in different stenosis groups (P < 0.05). INTERPRETATION: We concluded that the conventional and new indicators such as axial stress, critical bent buckling pressure, critical buckling torque could be useful for evaluating atherosclerosis development and also, may provide more information for physicians and interventional radiologists in designing strategies for decreasing risk in interventional treatment such as stent replacement and differentiation of vulnerable plaques.

Air pollution exposure is among the most prevalent reasons for environmentally-induced oxidative stress and inflammation, both of which are involved in the development and progression of central nervous system (CNS) diseases. Ultrafine particles (UFPs) plays an important role in global air pollution and the diesel exhaust particles (DEPs) are the most important component in this regard. There are more than 40 toxic air pollutants in diesel exhaust (DE), which is one of the main constituents of an environmental pollutant and including particulate matter (PM) especially UFPs. Thus, in this study, adult female and male NMRI mice were exposed to DEPs (350-400 µg/m3) for 14 weeks (6 h per day and 5 days per week). After 14 weeks of exposure, expression of pro-inflammatory cytokines (IL-1α, IL-1ß, IL-6, TNF-α), nNOS, HO1, NR2A, and NR2B and malondialdehyde (MDA) level were analyzed in various brain regions such as the hippocampus (HI) and olfactory bulb (OB). Exposure to DEPs caused neuroinflammation and oxidative stress in female and male mice. That these effects observed in females were less pronounced than in male mice. The male mice emerged to be more susceptible significantly than the female mice to induced neuroinflammation following DEPs exposure. Also, our findings indicate that long term exposure to DEPs results in altered expression of hippocampal NMDA receptor subunits, and suggests that gender can play important role in the modulating susceptibility to neurotoxicity induced by DEPs exposure.

OBJECTIVES: Common carotid artery (CCA) remodelling in the atherosclerosis process is an inherent necessary element that decreases the progress of significant lumen compromise. The present study used a semi-automated method to assess relationships of intima-media thickness (IMT), lumen diameter (LD) and inter-adventitial diameter (IAD) using ultrasound B-mode images of atherosclerotic carotid artery. METHODS: In the cross-sectional study, 120 male subjects (age range: 40-60 years) were classified into four research groups namely control, mild, moderate, and severe stenosis. The maximum near and far wall IMT, mean of both walls' IMT and IAD, and also LD of the left CCA were extracted for all participants. Pearson correlation coefficient was utilized to investigate relationships of IMT, LD, and IAD. RESULTS: Results revealed that the maximum far and near wall IMT, mean of both walls' IMT and IAD in the CCA were significantly different in stenosis patients and the control group (p< 0.001). However, there were no significant differences among the four studied groups in terms of LD of CCA (p = 0.65). There was a stronger correlation between mean of both walls' IMT and IAD in comparison with mean far wall IMT and IAD (p < 0.001). CONCLUSIONS: Results indicated that changes of IAD in the left CCA were associated with carotid deformation, and thus it can be considered as a predictor of atherosclerosis process.

This study aimed to identify instant intima-media thickness changes (ΔIMT) in the common carotid artery (CCA) during cardiac cycle in order to assess atherosclerosis progression. Using a computerized semi-automated method, instant IMT changes were extracted in the two walls of the left CCA (240 consecutive patients) using B-mode ultrasound images. We found that CCA ΔIMT increased from 8 ± 4% of IMTmax in the controls to 15 ± 6% of IMTmax in the severe stenosis group. According to the multiple ordinal regression analysis, ΔIMT was associated with the severity of carotid artery stenosis (odds ratio [OR], 4.95; p < 0.001), independent of sex (OR, 1.11; p = 0.04), age (OR, 1.14; p < 0.001), body mass index; OR, 1.13; p = 0.036), hypertension (OR, 2.04; p < 0.001), diabetes (OR, 1.38; p = 0.045) and hyperlipidemia (OR, 1.54; p = 0.002). We concluded that increment of CCA ΔIMT during the cardiac cycle was strongly and independently associated with severity of carotid artery stenosis or atherosclerosis progression.

BACKGROUND: Compton-scattered photons transfer incorrect spatial information. These photons are detected in used photo-peak energy window. In this study, three scatter correction procedures including dual-energy window (DEW), three energy window (TEW), and new approach were evaluated, and then the best procedure based on simulation and clinical conditions introduced. MATERIALS AND METHODS: In this study, simulation projections and three-dimensional nonuniform rational B-spline-based Cardiac-Torso phantoms were produced by GEANT4 application for emission tomography simulation code. For clinical study, 2-day stress/rest myocardial perfusion imaging (MPI) protocol was performed with 99m Tc-sestamibi for 46 patients. Image quality parameters including contrast, signal-to-noise ratio (SNR), and relative noise of the background (RNB) were evaluated. RESULTS: The simulation results showed that contrast values for DEW, TEW, and new approach were (0.45 ± 0.07, 0.5 ± 0.08, and 0.63 ± 0.09), SNR values (4.74 ± 0.94, 5.58 ± 1.08, and 6.56 ± 1.24), and RNB values (0.33 ± 0.06, 0.33 ± 0.07, and 0.33 ± 0.05), respectively. In clinical study, the contrast values for DEW, TEW, and new approach were 0.53 ± 0.03, 0.57 ± 0.07, and 0.62 ± 0.04 in rest MPI and were 0.52 ± 0.04, 0.57 ± 0.06, and 0.6 ± 0.05 in stress MPI, respectively. Moreover, for the rest images, the SNR values were 7.65 ± 1.9, 9.08 ± 2.2, and 10.2 ± 1.75 and for stress images were 7.76 ± 1.99, 9.12 ± 2.25, and 10.17 ± 2.04, respectively. Finally, RNB values for rest and stress images were 0.12 ± 0.03, 0.13 ± 0.03, and 0.13 ± 0.03, respectively. CONCLUSION: The simulation and the clinical studies showed that the new approach could be better performance than DEW, TEW methods, according to values of the contrast, and the SNR for scatter correction.

In this study a semi-automated image-processing based method was designed in which the parameters such as intima-media thickness (IMT), resistive index (RI), pulsatility index (PI), dicrotic notch index (DNI), and mean wavelet entropy (MWE) were evaluated in B-mode and Doppler ultrasound in patients presenting with carotid artery atherosclerosis. In a cross-sectional design, 144 men were divided into four groups of control, mild, moderate and severe stenosis subjects. In all individuals, far wall IMT, RI, PI, DNI, and MWE of the left common carotid artery (CCA) were extracted using the proposed method. Our findings showed that the maximum far wall IMT, RI, PI, DNI in the CCA were significantly different in the patients with mild, moderate, and severe stenosis compared to control group (p-value < 0.05), however, there were no significant differences in MWE among the four groups (p-value > 0.05). The proposed method can help physicians to better identify patients at risk of cardiovascular diseases.

BACKGROUND: Ultrasound measurement of carotid Intima-Media Thickness (IMT) is a suitable method to evaluate subclinical arteriosclerosis. OBJECTIVES: The current study aimed to present a new computerized algorithm to detect instantaneous changes of the IMT to Common Carotid Artery (CCA) of IMT in sequential ultrasound images by applying the maximum gradient and the dynamic programming. PATIENTS AND METHODS: In a cross-sectional design, an examination was performed on thirty healthy human subjects with the mean age of 44 ± 6 years from April 2013 to June 2013 in Beheshti Hospital, Kashan, Iran. In all individuals, the instantaneous changes of the far wall IMT on the CCA were extracted. Local measurements of vessel intensity, intensity gradient, and boundary continuity were extracted for all of the sequential ultrasonic 2D-frames throughout three cardiac cycles. The Pearson correlation coefficients and Bland-Altman analysis were performed to assess the relationship and agreement between IMT measured by the proposed and conventional manual methods. RESULTS: There was no significant difference between the proposed and manual methods with paired t-test analysis (in systole: 0.57 ± 0.10 vs. 0.56 ± 0.10 mm; P = 0.188 and in diastole: 0.63 ± 0.16 vs. 0.62 ± 0.10 mm; P = 0.122 for the manual and proposed methods, respectively). The Pearson correlation coefficients were r = 0.94 and r = 0.93 for IMTs and IMTd, respectively (both P < 0.001). Limit of agreements were narrow and considerable agreement was found between the two methods. CONCLUSIONS: The present study demonstrated that the proposed computerized analyzing method can provide accurate measurements of the IMT of the CCA in sequential 2D ultrasonic images.

BACKGROUND: Ultrasonic evaluation of intima-media thickness (IMT) is an early marker of assessing the development of atherosclerosis and determining cardiovascular risk. To attain the best possible diagnosis, it is essential that medical images be clear, sharp and without noise and artifacts. OBJECTIVES: Comparison of speckle reducing anisotropic diffusion (SRAD), discrete (DTD) and continuum topological derivative (CTD) on B-mode ultrasound images of common carotid and brachial arteries throughout the cardiac cycle. PATIENTS AND METHODS: In a cross-sectional design, an examination was performed on forty-two human subjects with a mean age of 44 ± 6 years from April 2013 to June 2013. This study was approved by the ethics committees of Kashan University of Medical Sciences and Beheshti Hospital. An ultrasonic examination of common carotid and brachial arteries of forty-two human subjects was performed. The program was designed in MATLAB software to extract consecutive B-mode images and apply region of interest (ROI) on the IMT of the common carotid and brachial arteries. Then, three different noise reduction filters with the Canny edge detection were used in ROI separately. Finally, the program measured the image quality metrics. RESULTS: According to values of eleven different image quality metrics (mentioned in the main text), there was a significant difference between CTD, DTD and SRAD filters with the Canny edge detection status in the common carotid and brachial arteries throughout the cardiac cycle (all P values < 0.001). For example, peak signal to noise ratios (PSNR) using CTD, DTD and SRAD filters were 95.43 ± 0.64, 88.86 ± 0.82 and 73.02 ± 0.20 in common carotid and 96.39 ± 1.25, 92.58 ± 0.11 and 88.27 ± 0.63 in brachial arteries, respectively (both P values < 0.001). CONCLUSIONS: By measuring image quality metrics, this study showed that DTD and CTD filters with the Canny edge detection respectively, are better than SRAD filter with the Canny detection for speckle suppression and details preservation in both arteries in the ultrasound images.

BACKGROUND: Common carotid artery (CCA) ultrasound with measurement of intima-media thickness (IMT) is a safe and noninvasive technique for assessing subclinical atherosclerosis and determining cardiovascular risks. Moreover, the pattern of wall thickening in the brachial artery (BA) is rather diffuse compared to the carotid artery and may be a more sensitive indicator of long-term systemic exposure to risk factors. Therefore noninvasive evaluation of mechanical parameters changes of both arteries has gained the attention of researchers. OBJECTIVES: The aim of this study was to compare different edge detection techniques with speckle reducing anisotropic diffusion (SRAD) de-noising filter in ultrasound images of both arteries. PATIENTS AND METHODS: In a cross-sectional design, an examination was performed on ten men with mean age of 40 ± 5 years from September 2012 to March 2013 through random sampling. An ultrasonic examination of the left CCA and BA was performed. The program was designed in the MATLAB software to extract consecutive images in JPEG format from the AVI. Another program was designed in the MATLAB software to apply regions of interest (ROI) on the IMT of the posterior wall of common carotid and brachial arteries. Next, different edge detections and SRAD filter were applied to the ROI, separately. Finally, the program measured mean-squared error (MSE) and peak signal to noise ratio (PSNR). RESULTS: The lowest values of MSE and highest values of PSNR were achieved by Canny edge detection with de-noising SRAD filter for IMT of left CCA and BA in 90 frames. CONCLUSIONS: Based on the result, by measuring the MSE and PSNR, this study showed Canny edge detection with SRAD filter is better than other edge detections in terms of speckle suppression and details preservation in CCA and BA ultrasound images.
